1. Primary Health Care Program
- Realm of Hope: Provides preventative care, such as physical exams, flu vaccinations, and prescription medicine.
- General Primary Care: Provides treatment of chronic illness, internal medicine, neurology, nephrology,
asthma, type II diabetes, hypertension and Marriage and Family practice.
- Gynecology: Provides annual pap smears.
- Oral Health Care: Provides oral exams, x-rays, cleaning, extraction, root canals, dentures, partials, fillings
and oral health education.
- Health Education: Provides smoking cessation, nutrition, personal hygiene and diabetes education and management.

2. Type II Diabetes Care Program:
Provides monthly provider visits, medications, dental visits, eye exams and glasses, foot exams, blood panels (1, 6 and
12 months in addition to routine lab visits) pap smears, EKG's, Glucometers and finger sticks. This program also includes
exercise and cooking education, including free educational materials, pre and post-testing and graduation awards upon
completion of classes.

3. Mi Corazon, Mi Vida Program:
(My Heart, My Life) Provides screening, medical care, case management, education, group support and lifestyle intervention
for women's heart health.

4. Precious Care for Precious Lives Program:
Provides first trimester medical care and education, including onsite provider and case manager, allowing up to three
medical visits per patient. The goal of this program is to reduce the amount of preterm births, low birth weight and
birth defects among newborns in Orange County.

5. Specialty Care Program:
Provides specialty care in the areas of urology, neurology, gastro intestinal, nephrology and ophthalmology.

6. Target Store Pharmacy Prescription Drug Program:
Provides patients with the opportunity to obtain prescription drugs at no cost or a shared cost of $4.00.
